Once an appointment is booked, PtP helps the patient complete intake before the first visit. PtP gathers the information the clinic needs early — insurance, health history, reproductive history, wellness questionnaires, and prior records — so the visit can start with a clearer picture.

Wellness Check-In

Physical health, emotional wellness, lifestyle context, support needs

Reproductive History

Prior cycles, pregnancy history, fertility goals, partner context

Medical History

Diagnoses, labs, imaging, medications, procedures, surgeries

Some information can be entered by the patient. Some may be retrieved from connected systems, where available, using FHIR / HL7 or clinic integrations.
